We would like to welcome you to our practice.
As a new patient we would like to inform you of the following. On the day of your appointment, please arrive 10 minutes early and bring with you your photo ID and insurance card.
We ask that all patients and accompanying family members refrain from wearing any cologne, perfumes, or scented lotions. Dr. Rahmanian is very sensitive and allergic to many fragrances.
Handicap accessible parking is located in the rear of the building. The google map link below can guide you to our location.
Please print the forms below by clicking on each link starting with "Patient" which would place the form on a new page for you to print. Please fill out the forms and bring them with you to your appointment. Please include all herbal and over the counter medications you may be taking.
Your first appointment is scheduled for 45 minutes. If you are unable to keep your appointment, then we ask that you give us 2 business day notice preferably or at the least 24 hours notice. If you do not show up for your appointment, don't cancel it ahead of time and it is not due to true emergency then you would be charged a 45 dollar no show fee prior to rescheduling. A second no show would result in no future appointments.
